Providence is calling a Pharmacist (Per Diem, variable shifts) to our Anticoagulation Clinicsin Everett, Mill Creek, and Monroe Washington.

We are seeking a Pharmacist, Anticoag to perform professional

pharmaceutical dispensing functions by providing prescribed medications, drugs,

and other pharmaceuticals according to standards and practices for patients in

all age groups including neonatal, pediatric, adolescent, adult and geriatric.

He or she will be expected to promote safe, cost-effective, rational drug

use and provide expert pharmaceutical consultation to other healthcare

colleagues.

In this position you will:

  • Communicate effectively bothorally and in writing.

  • Be able to establish andmaintain priorities for responding to requests and follow establishedpolicies.

  • Maintain confidentiality ofrecords or medical center information at all times; maintain patient andemployer confidentiality.

  • Perform research and analysisto provide financial data and recommendations for use in setting andrealizing financial objectives.

  • Ensure compliance with JCAHO,State and Federal regulatory agencies and PEMC standards and policies.

Qualifications:

Required qualifications for this position include:

  • Graduate of an accreditedschool of Pharmacy.

  • Pharmacist Degree.

  • Completed ASHP accredited PGY-1Residency Program or 3 years of acute care work experience.

  • Licensed as a RegisteredPharmacist in the State of Washington.

  • ACLS certification must beobtained within one year of hire.

  • Knowledge of community clinicalpharmacy practice including drug therapy standards, therapeutic outcomesmonitoring, drug use control, drug utilization review, formularydevelopment and management, adverse drug reaction management,pharmacokinetics, and nutritional support.

  • Knowledge of technical andquality assurance processes utilized in pharmacy practice, ambulatoryclinical pharmacy practice, patient care systems, and therapeuticcollaboration with prescriptive; knowledge with formulary and diseasestate management.
