Providence Health & Services Pharmacist - Inpatient in Everett, Washington
Providence is calling an Inpatient Pharmacist (1.0 FTE, Variable shifts) to Providence Regional Medical Center Everett in Everett, WA.
We are seeking a Pharmacist to perform professional pharmaceutical dispensing functions by providing prescribed medications, drugs, and other pharmaceuticals according to standards and practices for patients in all age groups including neonatal, pediatric, adolescent, adult and geriatric. The Pharmacist will promote safe, cost-effective, rational drug use and provide expert pharmaceutical consultation to other healthcare colleagues, while using Pharmacy & Therapeutics Committee approved protocols, prescribe narrow therapeutic index medication, and ensure appropriate follow-up.
In this position you will:
Direct the control of medication administration and drug distribution for inpatient and outpatient services for patients in all age groups including neonatal, pediatric, adolescent, adult and geriatric.
Participate in the development, implementation and monitoring of a pharmaceutical care plan to reach a desired outcome for each patient.
Promote cost-effective guidelines and individualized drug therapy.
Follow written protocols within the Washington State Department of Health regulations to determine appropriate individual therapy.
Minimize or prevent the incidences and severity of adverse drug effects.
Provide oversight and direction to staff.
Provide drug information and education (written or oral) to patients and other customers.
Maintain confidentiality of records or medical center information at all times.
Required qualifications for this position include:
Doctorate of Pharmacy (PharmD) from an accredited school of Pharmacy
Completed ASHP-accredited PGY-1 Residency Program OR three (3) years of acute care work experience
Licensed as a Registered Pharmacist in the State of Washington
ACLS certification must be obtained within one (1) year of hire.
Clinical Pharmacy Practice of Pharmaceutical Care
Current knowledge and experience in drug therapy monitoring, drug information, drug utilization review, pharmacokinetics, nutritional support, adverse drug reaction monitoring, cardiopulmonary arrest.
Preferred qualifications for this position include:
- Residency trained & 3 years of hospital experience
