Providence Health & Services Night Shift Clinical Pharmacist in Tarzana, California
Providence is calling a ( Full Time / Night Shift ) Clinical Pharmacist to Providence Tarzana Medical Center in Tarzana, CA. Schedule will be four (4) overnights per week, including some weekends.
In this position you will:
Provide and coordinate pharmaceutical care services and collaborative drug therapy management for patients.
Serve as a resource to the patients, physicians, pharmacists, nurses and other members of the health care team.
Assure safe and effective medication use and demonstrate proficiency in delivering care to assigned age‑specific patient population (e.g., infants, adolescents, adults, geriatrics, and pediatrics) and participate in related continuing education medication use.
Required qualifications for this position include:
Doctorate of Pharmacy from an Accredited College of Pharmacy.
A Bachelor of Science in Pharmacy is acceptable for employees hired prior to the revision date of this Job Description.
Current California Registered Pharmacist’s license in good standing
Current LA City Fire Card within 90 days of hireBasic Life Support ( BLS ) within 90 days of hire.
Pharmacists with a hire date prior to 11/01/2012 will have 1 year to complete BLS requirement.Advanced Cardiac Life Support ( ACLS ) within 90 days of hire.
Pharmacists with a hire date prior to 101/01/2012 will have 1 year to complete ACLS requirement.
Preferred qualifications for this position include:
- 2 years acute care experience or a Pharmacy Residency equivalent to 3 years of pharmacy practice experience preferred.
